Form 5500

The annual report ERISA requires for most employer-sponsored benefit plans. Group health plans with 100+ participants on the first day of the plan year must file. Due July 31 for calendar-year plans, with a 2.5-month extension available via Form 5558. Penalties for late filing can exceed $2,000/day.
